Theory of Disaster, Part 3, The Deception - Believe in something. Well you cannot manage without it. Only truth will be the basis of good decisions. So a disaster will have belief in wrong ideas. NASA conducted a study of the first space shuttle accident and found agreement to be the problem. A room full of compromise.
The master race and survival of the fittest is another self-serving bias.
Popular is just that. It is often the context for establishing decisions. Is popular even in the best interest of the population?
Often even after a disaster the truth will be hidden in favor of a popular victor's story.
With a thesis it will explain. How well it explains will test it. And that will lead to the anti-thesis, and synthesis.
Disasters are a test of a wrong thesis.

Inconsistencies and giving up.

The Soviet Union versus China changes. People did not want a kinder new face of Soviet Communism they wanted tangible change. Not propaganda but successful improved lives. Credibility fix requires success.

Failure of mid ship rigidity and no compartmenting. The three holds are only separated by a screen, really weird. They needed to be completely sealed with plate and structural cross members. Also, the hatch seals needed to be a rigid uniform design but were sealed with hand clamps, just not a solution more of a band aide fix. It is little wonder the ship broke apart in the middle.
